Patient Info/Practice Policy

Our goal at the Dr. Hughes’s Holistic Wellness Center is to provide you with the highest level of personalized care. We are committed to helping you achieve optimal health.

Forget the rushed 5 or 10 minutes impersonal doctor visits where you leave with a prescription and more questions than when you came in. Dr. Hughes takes the time to learn about you, really listen to your concerns, and offer hope to help you heal.

Your initial visit with Dr. Hughes is 90 minutes. During that time you receive a thorough evaluation which includes assessment of family history, past medical history, current medical problems, risk factors for preventable diseases, nutritional history, toxic substance exposure history, history of current symptoms, and physical examination. Dr. Hughes then offers recommendations for individualized diagnostic testing and treatment options.

What to bring to your initial visit:

Forms Lab Work
We use electronic medical records and ask all new patients to fill out and return a completed Intake Form electronically prior the first visit.

 Copies of any recent labs or imaging that you have had done in the past 2-3 years.
Medications Support
A list of all medications and supplements that you are currently taking (please bring all bottles with you). Feel free to bring a friend or family member with you if you prefer.

The second visit (approximately 1 to 2 months after the initial consultation) is 60 minutes. It includes a detailed review of test results and the formulation of an individualized treatment plan. This plan typically includes nutrition, vitamins, herbal supplements, minerals, bio-identical hormones, medications, acupuncture, cupping, and lifestyle modification.

Subsequent follow-up visits are 30 minutes and are held 1 to 3 months after the treatment plan is implemented. Follow-up testing will be necessary to evaluate the effectiveness of the therapy.

Financial Policy

Integrative Holistic Medicine requires more lengthy and in-depth office visits than the standard 5-10 minute visit covered by insurance plans. Therefore, we are unable to directly bill your insurance company. However, as a courtesy, we will provide you with a receipt containing the necessary diagnostic and procedure codes that you may send to your insurance company for reimbursement. We operate as a self-pay clinic and full payment is due at the time of your visit. We accept cash, check or all major credit/debit cards.

Cancellation Policy

To cancel or reschedule an appointment you must contact our office 48 hours prior to your scheduled appointment time in order to not incur charges. Please note: Tuesday appointments require cancellation on the previous Friday. If you fail to cancel/reschedule within 48 hours you may be charged the full cost of the appointment. Exceptions maybe made for emergencies.

Scented Products Policy

We have many chemically-sensitive staff and patients in our office, please refrain from wearing scents (perfumes, colognes, etc.) when coming to the office.
