We would like to welcome you as a new patient of Dr. Hughes’s Holistic Wellness Center. We look forward to working with you to address all of your concerns, and we will do all we can to ensure you achieve the best possible results that you can. The confidence and trust you have placed in us is greatly valued.
Prior to your first visit, please complete the appropriate forms so we can speed up your registration and Dr Hughes has accurate information for your visit. We would appreciate it if you could arrive 10 min early to your appointment to complete registration.
For a new patient doctor visit, please fill out our new patient form. Our new patient form is lengthy (16 pages) so make sure to give yourself adequate time to complete it; Dr Hughes will go over the form in detail with you during your first appointment so don’t stress greatly about it.
If you are a new patient receiving only acupuncture or cupping services, please fill out the designated form below.
